By this question I don't mean to ask the origins of the name. It's apparent looking at today's match against itsy pause and past performances where he has failed miserably to convert numerous breakpoint chances that this nickname is no misnomer. With this discussion I hope to focus on two things:

1. Is Fed really as bad of a returner / breaker as he seems, or is that only in comparison to Djokdal?

2. Why is Breakpointerer a thing? Is he doing something technically wrong on returns in big moments or is he mentally weak in these moments?

Nothing wrong with his return
He just plays passive a lot of the times of return.

Most servers up their game when facing breakpoints but yes Federer is not cool enough when he gets breakpoints veering from being passive to overly aggressive. It’s moot, he was serving poorly and outgunned by Tsitsipas in the rallies. The result is not really unexpected.
