I think this situation has happened in ATP Tokyo sometime ago. If I remember correctly, the substitution at that time was Ferrer...
Yep, Tokyo 2007.

Also Bangkok 2007 and Beijing 2006 apparently!

This formulation has appeared in 2016 version of ATP Rulebook only. Previously (at least within 2013-2015) Rulebook had another point:

6) Emergency Situations.
a) The President may authorize substitutions in ATP World Tour tournaments in situations when there are withdrawal(s) of player(s) who would have been seeded based upon their position on the original acceptance list. The withdrawal and substitution must occur within the period beginning on the Monday prior to the start of the event week and ending at 12 Noon, Eastern Time, USA, on the Friday before the event week. The substitute(s) ranking, as of the entry deadline, must be within the original cut-off for the main draw. If no substitution is made within this defined time period, then the next player on the acceptance list shall be moved into the main draw.
In 2013-2015 on Challenger Tour the above point was a base for a few emergency substitutions, see for instance EL and MD at Fergana'2013 challenger http://www.menstennisforums.com/2873-2013/333377-week-39-fergana-winner-radu-albot.html
Don't remember any cases of applying this point on ATP World Tour in 2013-2015

You guys, Mahut was always gonna play this event. Again... Unless something bad happened. Tournament actually announced that he was coming back to defend his title back in mid-April. Dude just forgot to sing up. He's French after all...

Well Copil has had his fair share of (nonsensical) WCs in foreign events, so I am not too sad about him not getting one. :p

Yeah. That too, though he's not even close to other players on this matter :)

And it would suck if that was the reason, regardless

Mahut: “I almost had to play qualies because I forgot to enter the tournament before the six-week deadline,” Mahut revealed. “I’ve only forgotten to do this twice in my career. The last time was 2000. I went online to check the entry list and could not believe I was not on it. So I called the tournament director and I am really happy I got a wild card.”
