Do Medvedev, Zverev and Tsitsipas converse in English or in Russian?

Medvedev is Russian. Zverev has Russian parents. Tsitsipas’s mom is Russian. All 3 speak fluent Russian.

Do they converse with each other in Russian or in English?

We know they don’t like each other much.

Med called him “a small kid who doesn’t know how to fight” in English, so my guess is that they speak English.

Zverev's mother tongue is Russian, despite being born in Hamburg/Germany. He and Medvedev probably speak Russian between each other.
Tsitsipas's 2nd language is Russian (from her mother), he can switch to it, but my guess is that he chooses English vs Medvedev/Zverev.

That's why country representation doesn't matter in tennis anymore. Davidovich Fokina also has Russian ties and then you have the "Australian" players who are actually Greek. So Tsitsipas could represent Russia and both Kyrgios and Kokkinakis could represent Greece, but yes this has nothing to do with OP asked :) But the world is dominated by Russian players if you look in detail, and we haven't yet talked about women's tennis...

I think it depends on the circumstances. When there's someone else who doesn't understand Russian in their conversation, then they should be conversing in English. When they're conversing amongst themselves only, they're most likely to be speaking Russian. I don't have any evidence to present here, but I'm quite certain of that.
