He looked nervous serving, from the start of first set, but if this was a mental timeout, well, very sad.
But most important is that Fed keeps playing and not getting distracted

Panic attacks have physical manifestations as well - quicker heartbeat, sweat, feeling cold, trembling, might even go as far as blackout.

Very strange breakdown from Cilic - his fall in the first set didn't look that bad (no rolled ankles or anything)... and Cilic still moves fine and doesn't really look to be in pain either.

I think it was just a mental breakdown. Roger was so clutch in the first games of set 1 when Cilic had his opportunities and from then on took control. Cilic looks fine now, but Roger's just playing really well right now.
