5 Tiebreak match

I was wondering whether there has ever been a match in history with a scoreline such as:
7-6 6-7 7-6 6-7 7-6, going to 5 tiebreaks.
I know: it could only happen at the U.S. Open, but has it ever happened since the invention of the tiebreak?
I know that Sampras had beaten Agassi once at the Open in 4 tiebreak sets, but I cannot recall any match which went to 5.

Also, just wondering: What is the record for the longest tiebreak ever (It's got to be 70-68, lol).

I think the longest tie break ever played was at a future qualification match. It was something like 36-34, don't know the players.

Not sure about this, I think Karlovic-Stepanek in 2009 DC F is worth a mention though, that was 4 TB sets and the 5th set 16-14, so it would have been 5 TB if the format was like USO or the current DC.

Wayne Arthurs won 3 match in 4 Tie Break sets :

Wimbledon 2001 : Arthurs def Dent 7-6(2), 7-6(3), 6-7(4), 7-6(5) (no break in the whole match)
Roland Garros 2001 : Arthurs def Coutelot 7-6(5), 6-7(5), 7-6(5), 7-6(4)
Wimbledon 1999 : Arthurs def Santopadre 7-6(7), 6-7(5), 7-6(10), 7-6(4) (no break in the whole match)

As far I know, no matchs went to 5 TB.
It could only happen at the USO, I guess one of the closest match from it was Kriek against Tanner in 1983 (6-7, 3-6, 7-6, 7-6, 7-6).

Last year in the Australian Open, Lopez def Pella 7-6, 6-7, 7-6, 6-7, 6-4
