It's not a really interesting stat, there are playing that just roll the serve in with minimum risk. I'm pretty sure there have been matches where player had 100% first serves, I believe Nadal came close a few times.

Paradoxically an usually high first serve percentage is often a sign of an injury preventing a player going for his serve with full commitment and risk.

Indian Wells 2002, R64, Kucera vs Lapentti 6-2 6-3
Palermo 2003, R32, Montanes vs Ramirez Hidalgo 6-2 6-4



The next best would be 98,07692% of Agassi (50/51) vs Braasch in Scottsdale semi 1994.

notes:
- I left out the early retirements when players retired with 5/5 1st serves retired and also had 100%.
- the stats for the Kucera vs Lapentti match are likely wrong because both served 39/39 and 40/41, respectively. Would be an incredible percentage, especially for Kucera.

According to ATP and not counting early retirements, the lowest percentage was by Tom Nijssen with 10/47 (.21286 %):

Lyon 1991, R32, Sampras def. Nijssen 6-3 6-2
